Which services are best for me?

If you have a medical condition that requires medical nutrition therapy (MNT) then we suggest that you book in for a dietetic consultation, which uses established dietetic practices for treating health conditions.


If you're interested in treatment methods and programs that are holistic,  incorporate wellbeing, herbal supplements and functional health testing, then a naturopathic consultation is most suitable for you.

How many sessions will I need?

If you'd like to chat about whether we're a good fit for your needs we offer free discovery calls. You can book online for a 10 minute phone chat.


3-5 consultations would be a good start for chronic health problems of 6 months or more.  


For long standing and complex health problems then a 12 month membership is available for coaching and support.

How do I make an appointment?

An Initial Consultation is the step in which we assess your needs and goals, identify health priorities to treat, develop a plan of action, and draft a treatment program.


We then follow-up for your response to the treatment plan, arrange for further tests and investigations (if needed), and review how the plan is working for you. 


As you progress we then transition to maintaining your progress and setting you up for the future.

It's easy - simply click on BOOK IN from the menu above where you'll be taken to the online booking system.  


You'll receive an SMS confirming your appointment and an email with a link to the Pre-Assessment Form which gathers information about your health and helps us to prepare for your initial consultaiton.


You can also get ahead by clicking on FORMS from the menu above and you can access the PRE-ASSESSMENT FORM directly.

Medicare provides rebates only for dietetic services provided under a Medicare Chronic Disease Management Plan or an Eating Disorders Management Plan. Your GP can determine your eligibility.  (Naturopathic services aren't available under Medicare.) 


 Please note that we no longer offer bulk billng for Chronic Disease Management Plans or an Eating Disorder Plans.  


You will however be able to claim a Medicare rebate if you have this type of referral.

 Some health funds offer rebates for Dietetics under their Extras Cover, however this varies so please check with your fund.


Unfortunately naturopathy is no longer supported by private health cover so rebates aren't available for naturopathic services.


Private health fund claiming is available in-clinic via Healthpoint.  You can check the list of participating health funds below.


Gap payments can be made via EFTPOS or credit card.

Nutritionpath is committed to providing quality services to you and this policy outlines our ongoing obligations to you in respect of how we manage your Personal Information.


We have adopted the Australian Privacy Principles (APPs) contained in the Privacy Act 1988 (Cth) (the Privacy Act) which govern the way in which we collect, use, disclose, store, secure and dispose of your Personal Information.

Collection and use of personal information

The personal and medical information collected from you is primarily for health assessment, advice and counselling.    

The personal information you provide may be used in corresponding with others involved in your health care including your doctors and other health professionals.  

It may also be used for administrative purposes such as to remind you of upcoming appointments, billing or payment advice.

You may decline to have your health information used in all or some of the ways outlined above however it may influence our ability to manage your health care to provide the best outcome for you.


Email and text message communication

Nutritionpath offers clients/guardians the opportunity to communicate by email and text message (text).  

There are however risks of communicating by email and/or text which include but are not limited to:

•  Email and text can be circulated, forwarded and stored in paper and electronic files.
•  Backup copies of email/text may exist even after the sender or the recipient has deleted his or her copy.
•  Senders can easily type in the wrong email address or phone number.
•  Email and text may not be secure and therefore possible that the confidentiality of such communications may be breached by a third party.
•  Email and text can be used as evidence in court.

Clients are responsible for:

•  Informing Nutritionpath of changes to their email address or mobile phone number.
•  Informing Nutritionpath of the types of information that they do not want to be sent by email or text.
•  Protecting their password or other means of access to email or text.

Nutritionpath will:

•  Use reasonable means to protect the security and confidentiality of email and/or text information sent and received, but will not be liable for breaches caused by clients or a third party.
•  Respond promptly to an email/text from the sender however cannot guarantee response within a particular time.  Thus, email/text should not be used for urgent or emergency situations.
•  Take reasonable steps to ensure client privacy and confidentiality throughout all methods of communication and correspondence.

 

Security of Personal Information

Your Personal Information is stored in a manner that reasonably protects it from misuse and loss and from unauthorized access, modification or disclosure.

When your Personal Information is no longer needed for the purpose for which it was obtained, we will take reasonable steps to destroy or permanently de-identify your Personal Information. However, most of the Personal Information is or will be stored in client files which will be kept by us for a minimum of 7 years.

Access to your Personal Information

You may access the Personal Information we hold about you and to update and/or correct it, subject to certain exceptions. If you wish to access your Personal Information, please contact us in writing.


Nutritionpath will not charge any fee for your access request, but may charge an administrative fee for providing a copy of your Personal Information.


In order to protect your Personal Information we may require identification from you before releasing the requested information.
